Usually this indicates a system load issue or database problem. It can also be caused by poor network connectivity between the agent and ViciDIAL.

Dear geoff3dmg,

Thanks for quick reply.

Can you tell me how to get log to notify the user.

or any condition that it will tell that its database or network problem since it happens in rare case.

Please help me with that .

System load -> use 'top' on the server console. If the load is higher than 75% of the number of cores in the machine you'll have a problem.
Database -> use 'mysqladmin processlist' on the server console. Check for any queries taking too long (more than a couple of seconds).
Network -> use 'mtr' on the server console. Try several different IPs (eg multiple agent PCs in different locations, SIP providers, etc). Your looking for high latency and/or packetlioss,
