Vicidial not showing leads

Hi guys

I cannot insert new leads, when a try to load a lead with the "Add a New Lead" option i get this message: ERROR: Lead not added, please go back and look at what you entered
and if i try to load through the "Load New Leads" options i cannot see the vicidial columns.

Also the leads already loaded are not showed in the list, all lists show 0 leads but the registers are in the database.

What can i do?
