Dialing a directory or extension after connecting

I wanted to know if anyone is doing something different to dial extension numbers after connecting to the main number. We currently dial the extension number or menu option on the softphone. Doing this seems to interupt the current session and we start experiencing noice and interference on subsiquent calls.

Asterisk version?
astguiclient version?
did you apply the DTMF passthru patch?
Do you have the "F" flag on your meetme entries in extensions.conf?
What kind of softphones?
What is teh DTMFmode set for the softphone accounts in Asterisk?
