Looking for a common component in case this is a cross-connect issue or a bad configuration on a specific server.
If provided several examples, we can likely find the problem in an hour. If you find the problem, it *may* be as simple as using perl install.pl after updating the asterisk version on the offending server. Note that some vesions in the astuiclient.conf file during installation omit the ".X" in "11.X" and that .X is require DURING installation. If it's not there, adding it later still requires re-running perl install.pl with the sample conf file option included so the correct (new) sample files are invoked.
