Apache and MySQL Problem

I am setting up a new Vicidial environment and I want to test out Vicidial 2.05

I have a Apache/Asterisk box

and I have a MySQL box

I am getting this error on my apache login screens

I cant seem to find what I missed. I have tried flushing the IPtables but no luck

I added my asterisk server to my mysql servers hosts file and got past that error

now my new error
