Vicidial outbound predictive: Agent call not received

Hi,
Can any one help me? I am using ViciBox_v8.x86_64-8.0.1 and facing problem at outbound predictive dialling. Agent sometimes received call but cannot hear. Agent most of the times not get call while customer received the call from predictive outbound campaign but within 3-4 seconds call terminated saying good by. I have configured On-Hook=Y.

Dose agent get a call from soft phone while login?

This issue seems like the agent isn't logged into a conference during agent login. So the customer hears "conf-onlyperson.gsm" this tone.

OnHook = NO for outbound dialing.
