Custom Form Fields Error

Hello,
We currently have one campaign on our call center setup, but we would like to add some custom fields to the form tab for our agents to use.
For a test, we setup one choice box as shown below. We assigned it the ID '412', as well as assigning it to our one and only campaign.

On the agent screen, if the Agent were to click the Form tab, we are presented with the error:
"ERROR: no custom list fields table"

We've confirmed that the fields are showing up properly in the database, however, we cannot get the actual form to display them.

Is there a setting we're missing?

What was the list_id of the lead that the agent was on on their screen?

Was it a manual dial call, inbound call or auto-dial call?

Problem was solved.
Solution: Changing the ListID for the call menu.

@mflorell This custom field is only for auto dialer?

Or can we use i for Manual dial and incoming call too?

Custom fields works for any kind of call in VICIdial
